How to Lower Your Medicare Part B Premiums if You’re in the Top 8%

Most people on Medicare will pay the standard $185 per month for Part B in 2025.

But if your income is higher, you might face an extra charge known as IRMAA (Income-Related Monthly Adjustment Amount).

Roughly 8% of Medicare enrollees will pay this higher premium in 2025.

If you’re one of them, here’s what you need to know and how to prepare.

 

Why Some People Pay More

Medicare Part B premiums are based on your 2023 income (two years prior).

The higher your income, the more you pay.

This extra charge is called IRMAA, and it’s added to your monthly Part B premium automatically if your income exceeds certain limits.

2025 Medicare Part B IRMAA Brackets

  • Standard Premium: $185 per month

  • If your income (single) was $106,001 - $133,000 (or $212,001 - $266,000 joint), you pay $259 per month

  • $133,001 - $167,000 (or $266,001 - $334,000 joint): $370 per month

  • $167,001 - $200,000 (or $334,001 - $400,000 joint): $480.90 per month

  • $200,001 - $500,000 (or $400,001 - $750,000 joint): $591.90 per month

  • Above $500,000 (or $750,000 joint): $628.90 per month

 

What You Can Do if You’re Affected

If you receive a letter from the Social Security Administration (SSA) about IRMAA, don’t panic. Here’s what to do:

  1. Check the Notice: Confirm your income information is correct.

  2. Appeal if Needed: If your income dropped due to retirement, job loss, divorce, or another life change, you can file SSA Form SSA-44 to request a lower premium.

  3. Plan Ahead: Your 2023 income impacts your 2025 premium. Consider strategies like Roth IRA conversions, charitable distributions, or other planning to lower your MAGI for future years.

  4. Seek Assistance: If you’re struggling with premiums, Medicare Savings Programs and Extra Help may help cover Part B costs.
